VOLUNTEER FORM
You are required to have your chosen organization sign the PROOF OF VOLUNTEERING form providing evidence that you did indeed volunteer for a minimum of 2 hours.

This form is found in the RESOURCE BOX on your computer. The name of the PDF is "Proof of Volunteering".

Before you can receive a grade for this project you must turn in this form. No form, no grade.


IMAGE RELEASE FORM
No matter who you are filming, no matter where you are filming, no matter why you are filming, you must get image releases from everyone that will be on camera. The form "GHCHS license release" that all persons on camera must sign indicates that he or she has consented to have their likeness recorded and used for non-commercial purposes. We have releases you can take with you. They are available in the RESOURCE BOX in the PSS folder. Make sure you always have releases with you when you're out shooting (print a handful out well before your first shoot and carry them with you).  If you encounter someone who will not agree to sign the form explain to them that it is for your school and for a good cause. If they decline then find someone else to record.

You do not need to obtain releases during your fact-finding and pre-interview unless you are actually recording video.  You only need to complete the form for those persons appearing on camera.

Make sure to present the releases to your subjects before you start shooting. No one will appreciate the fact that you surprised him or her with a release after the fact.

If you are dealing with a corporate entity let your contact person there know that you will require a signed release for on-camera interviews and that you would be happy to forward one along for their approval. If they request a change tell them you will get right back with them after you get clearance for the change from your filmmaking teacher.

If you find that you've forgotten the image releases, you can always go back later and ask them to sign post-shooting.

In situations where there are a large number of people (such as an open public gathering) and it would be difficult to get everyone to sign these releases, it is appropriate to post a conspicuous sign, or signs, stating that the proceedings will be recorded and that by entering the space they are consenting to have there likeness recorded. An appropriate sign (PUBLIC GROUP RELEASE) is available in the RESOURCE BOX. If you encounter a situation such as this make sure the organizers of the event are aware that these signs will be posted. Make sure to post the sign (or signs) conspicuously just outside the room or area where the meeting is to take place.

If there is an area of the room where attendees will not appear on camera you need to indicate this verbally and/or by signage. They may wish not to attend if there is no non-recording area; it is their choice.
